Insured Patients

Please check with your insurer that you have adequate cover. We can help you with this, but only your insurer can confirm your cover is adequate.

- If your insurance company offers a direct settlement scheme, Q Physio Ltd will send the account and claim form to your insurer for payment. In order to do this, you will need to obtain a Pre-Authorisation code in advance of treatment. If your insurance company does not pay the amount in full, you are responsible to pay any outstanding balances.

Employer/ Third Party Payment

- If your employer or Third Party has agreed to pay your fees, a letter of authority must be sent to Q Physio Ltd in advance of your treatment. All treatment fees must be paid within 24 hours of each appointment.

Uninsured Patients

- If you are not covered by medical insurance, you must pay your treatment fees in full after every appointment.

Cancellation and Non-attendance Policy

For any appointment booked with Q Physio Ltd, the following cancellation and non-attendance policy applies: - If you cancel an appointment with less than 24 hours’ notice, a fee of 50% of the treatment price will be incurred.

- If you fail to attend an appointment without cancelling, a fee of 50% of the treatment price will be incurred.

- If you fail to attend 2 or more appointments without cancellation, you will incur the total cost of your appointment.

Payment Terms and Conditions

Q Physio Ltd will charge your credit/debit card in the following situations:- Where you cancel your appointment within 24 hours’ notice, a fee of 50% of the treatment price will apply.

- To pay for your treatment in full if your insurer does not deal directly with Q Physio Ltd.- Expenses not covered in whole or in part by the insurance company.

Discount Codes

If you have a Q Physio Ltd discount, please note that unless stated otherwise, only one discount can be used per treatment.

Clinical Consent

When attending your appointment, you will be asked to complete a Patient Registration Form. You will be asked if you can confirm that you have read and understood the following and give your informed consent to Physiotherapy/Osteopathy/Sports Massage/Personal Training appointment and (where necessary) subsequent treatment/appointments:

- I confirm that I understand I can have someone accompany me to treatment session(s) should I wish.

- I confirm that I understand that I can bring an interpreter to my Physiotherapy session(s) if I wish.

- I confirm that I understand that I may be required to remove certain articles of clothing to enable appropriate assessment and subsequent treatment (where necessary).

- I can confirm that I understand that my Health Professional may need to write a clinical report if I need to be referred to a Doctor.

- I confirm that I understand that I can stop the Physiotherapy assessment or treatment process at any time should I wish.

- I confirm I agree with the payment terms and cancellation policy in place.
